Disclosure and authorization for release of information for volunteers (non-financial) 

As a part of our background check and investigation, the Evangelical Lutheran Church in America, CBCInnovis, or another consumer reporting agency may prepare an investigative report prior to your acting as a volunteer. The consumer investigative report may consist of contacting all listed references and congregations where you have been a member. It may also include a check of applicable criminal, police, and court records. 

Below you will find an authorization and release for the ELCA, CBCInnovis, or a consumer reporting agency to prepare and for the ELCA to receive a copy of that report. 


Authorization and release to obtain information

Under all applicable federal, state, and local laws, I hereby authorize and permit the ELCA to obtain from CBCInnovis or another consumer reporting agency an investigative report which may include the following:

Records concerning any driving, criminal history, criminal records, civil records, and drug testing; I agree that a copy of the authorization has the same effect as an original.

I hereby release and hold harmless any person, firm, or entity that discloses or receives matters in accordance with this authorization, as well as the ELCA, CBCInnovis, or another consumer reporting agency used by the ELCA from liability that might otherwise result from the request for use of and/or disclosure of any or all of the foregoing information.

I do hereby agree to forever release and discharge this Company, its agent, CBCInnovis Employment Screening Services, and their associates to the full extent permitted by law from any claims, damages, losses, liabilities, costs and expenses, or any other charge or complaint filed with any agency arising from the retrieving and reporting of information. According to the Federal Fair Credit Reporting Act, I am entitled to know if employment was denied based on information obtained by my prospective employer, and to receive, upon written request, a disclosure of the public record information and of the nature and scope of the investigative report.
